Inspector advised us an inactive coal processing facility had steam or smoke coming from coal pile. We dispatched people to investigate, found steam or smoke, moved equipment to spread material over a large surface, applied water to ensure no fire. Having found no fire we kept applying water to keep down the possibility of a fire.

EE and other EE were attempting to open a gate valve on the heavy media suction line when EE's thumb was caught between the wheel on the valve and the valve itself. EE's thumb was dislocated and a mobility limiting brace was applied by the orthopeadic physician.EE has not lost any time from work.

The #1 MRS was 150 inby spad #635 when it became disabled and would not tram. After attempting to correct the problem the roof conditions deteriorated and fell in on the MRS. The fall area was approximately 20'x20'x 10 feet thick. No persons were affected by the fall. MSHA was contacted and monitored the recovery process.

Employee had drilled a hole and stuck the roof bolt in the hole and was preparing to push the bolt up. The bolt didn't align with the wrench so he was going to bump the rotation and caught his glove around the roof bolt. He received 22 stitches to his left hand. EE stated that he was walking back toward the miner after conducting an air reading when he slipped on a rock and fell into the gear case of the miner. He was treated at MAR Hospital for a laceration to the forehead requiring 7 stitches. EE returned to work on the next shift at full duty.
