My cell phone flew off of the top of my car while driving and shattered. I drove by the place on the road a number of times to try to find it but I never found it. I went to a XXXX store in XXXX, KY to get a " loaner phone '' and was told they don't do that anymore and that I'd have to pay the complete bill and purchase a new phone in order to get service. I inquired about spreading it out month to month and they said No.
I got a XXXX in the interim while attempting to work this out, paying on my XXXX bill the whole time.
When, later, I noticed that my cell phone number was being used and/or listed by a couple in XXXX, XXXX Their information is disclosed in the attached police correspondence. I also informed an executive at XXXX.
I am not paying a remaining bill on someone else 's use of my cell phone number. For all I know, they found my shattered cell phone on the side of the road and claimed it and began listing it and using it as their own. You can XXXX their information and it is all there.

My company credit card bill, originally from XXXX XXXX, has been sent to collections through Nationwide Credit , Inc and I have received the collections statement. I went on a company issued training trip to XXXX, XXXX and was informed to use the card for flight tickets, room and board, food and XXXX testing to reenter the United States and to fly. I also had to cover payments for another employee on the trip, who did not get card authorization prior to travel, so their card was declined. The company let me go and has now discontinued paying the credit card payments.
I notified the original creditor multiple times that I was not responsible for the bill, and that the debt was not valid under my name, as it was a company card and issued trip in which I was asked to use the card. I was also required to assist the other employee financially on the trip. The original creditor continued to call, harass, and got very unprofessional with me, threatening me to pay the debt that was not mine. I let them know I did not agree with the debt and asked them to contact my employer for payment. I also attempted to contact my employer. However, they ignored all communication and still sent the account to collections under my name.
The company required that I put my name on the account for the training trip, even though I did not desire to do so. No effort was made by either party to contact my employer and rectify the bill in that manner as their name is also on the credit card.

This apartment complex added Debit on my credit report for {$3400.00}. I never lived there in my life I never lived in that city were the apartment complex located. XXXX XXXX in XXXX Kentucky. I tried to dispute multiple times through my credit bureau. I have all evidence to support that Ive been living where I currently live for the past six years of my life. I need this removed please. Its affecting my credit score big time.

I started getting medical bills from XXXX Florida medical center. Which i have never been to any sort of medical center in florida. So i called the number on the bill and tried to explain it has to be a mistake. They did not offer to help at all said that it was me and i need to pay the bill. So I contacted the Hospital directly and explained it could n't have been me i was working at my job in Indiana. So they were helpful and told me i needed to prove it some how to clear things up. So i had sent a Picture of my I.D as well as a signature to the hospital. I received a phone call stating that there was a mix up at the hospital someone with the same name and birthday as me was there but they lived in Florida. I stopped getting bills from the hospital. But as of today i received a bill from Transworld Systems for a XXXX medical bill. I will provide any documents needed to get this resolved for good! Just do not want my credit impacted over bills that are not mine. Thank you

Transworld Systems Inc sent me a debt collections bill in the amount of {$64.00}. I do not owe this alleged debt. I have called Transworld Systems Inc, and they have used abusive and hostile communication tactics. In addition, the Transworld Systems customer service representatives have repeatedly hung up on me multiple times when I have called them. Transworld Systems Inc has repeatedly violated the Fair Debt Collections Practices Act with their repeated abusive and hostile debt collections tactics towards me. I do not owe this alleged debt nor do I want any further communication from Transworld Systems Inc as they have repeatedly violated the Fair Debt Collections Practices Act with their abusive and hostile debt collection tactics.

I never received anything from XXXX XXXX XXXX apartments. Now I see Transworld Systems is reporting this issue on my credit file. I did a dispute to the credit bureaus and a direct letter to Transworld Systems asking for validation. The bureaus mailed me that Transworld verified as accurate, but no response from Transworld.
I asked for validation of the {$2800.00} current balance amount. An itemized statement showing all charges and fees. The date when I vacated the apartment. A list of any damages.
Plus according to Ky. Rev. Stat. Ann. 383.670, I would like to know what steps were taken to re-rent the unit to limit my exposure to additional rent costs?
