EE & co-worker were pulling piece of conveyor belt into new belt drive. The employees had installed a pulley onto a belt hanger. A steel cable was inserted through pulley & anchored onto conveyor belt being installed. As conveyor belt was being pulled, belt hanger broke allowing slack in steel cable which resulted in EE being struck in the back of the head with the steel cable.

As he was going around, EE's left side wheels dropped off the E bound shoulder. Due to the steep grade, this caused him to overturn onto its L side, coming to rest on the E bound shoulder.
