A non-injury roof fall was found by mine examiner. The fall was located 1cc inby spad #6654 on the 1-left bleeder panel. J order was issued later modified to a K order to allow additional roof support to be installed. Area has been dangered off. This fall will not be cleaned up. This is a non-producing mine that is in the process of being shut down. Fall was 40'Lx18Wx8'H.

EE was tightening a turnbuckle on the #5 Head drive. As he was repositioning this arm, he struck a piece of metal on the oil bath of the drive resulting in a laceration which required 7 stitches.

A Roof Fall was discovered in a non-active area of the mine on 8-17-2011. The roof fall is located in the # 4 entry between crosscuts # 3 & 4 approximately 40' feet inby survey station # 7459 of the 2 Left Panel off the 1-East submains. The roof fall is approximately 40' feet long x 18 1/2 wide x 6' high. The roof fall was reported to the MSHA Hotline on 8-17-2011.

EE was tramming feeder on belt move. Had to place crib blocks under it due to busted up bottom. Feeder slid off from crib blocks causing it to hit solid. EE's left ring finger was caught between lid and frame and pinched the end of his finger.

EE & co-worker were pulling piece of conveyor belt into new belt drive. The employees had installed a pulley onto a belt hanger. A steel cable was inserted through pulley & anchored onto conveyor belt being installed. As conveyor belt was being pulled, belt hanger broke allowing slack in steel cable which resulted in EE being struck in the back of the head with the steel cable.

This fall was called into the MSHA Hot line at 4:33 p.m. There was no entrapment, and no apparent life threats. Fall was 1 bk inby and 2 bks outby spad # 7560. Width and Height is unknown due to this being a pillar section.

The employee was helping to move the continuous miner and MRS cables during retreat mining while the continuous miner was moving back the machine accidentally hooked the MRS cable and pulled them down. When the cable was pulled down it caused the employee to fall to the ground injuring his left foot.

Unit 1 was traveling W on old trace road. He was approaching a paving crew which was paving the W bound lane. A flagger waved him around into E bound lane to go around the paving crew. As he was going around, EE's left side wheels dropped off the E bound shoulder. Due to the steep grade, this caused him to overturn onto its L side, coming to rest on the E bound shoulder.
