Employee was traveling from #3 belt line to #4 head drive on a Johnson Industries 2 man carrier, when a piece of rock fell from in-between the roof bolts hitting on the Johnson carrier. The employee had his right hand on the steering wheel and the rock struck his right hand and left leg. The rock measured 48 long 24 wide and 5 thick.

Employee was operating the left side of a dual boom bolter when he got his left hand entangled with the drill steel and a wrench. This resulted in an amputation of his left hand and arm near his elbow. He was treated on site and then taken by ambulance to a local hospital. He had surgery and his arm was reattached. He was released from the hospital on 4-4-08.
