The fall measured app. 60' long x 19' wide x 6'-8' high. This area is app. 800' from the #1 unit(MMU 001-0).

The section foreman was marking up sight lines in #2 entry of #1 MMU001-0, when a piece of rock fell between pins and struck him in the back. The rock measured 4' x 4' x 2. The foreman finished his shift and ensuing shifts, but began to miss work due to the injury on 10-05-2011.

EE was trying to remove a large rock from the tail of the Joy 14Cm15 by lifting the tail section to the roof and dragging rock off. A piece of the rock broke off and flew back 8'-10', striking the miner operator on the left lower leg and ankle.
