Citation text line 29 CFR 1917.45(f)(4)(iv): A prominent warning sign was not posted at the foot of a crane ladder or stairway when the top of a ladder or stairway or other position was located where a moving part of the crane, such as a revolving house, could strike an employee ascending or descending the ladder or stairway. a) On or about September 16, 2020 at the river terminal the overhead crane (Serial #CI-29638) was accessible by fixed ladder exposing employees to struck-by hazards and the employer did not ensure that a prominent warning sign was posted at the foot of the ladder.
