While hooking up a trailer to a lo-trac the employee asked the forks be raised. When the forks were raised, one fork came up quickly and came back down pinching the inside of the right leg between the fork and trailer causing a contusion. It was later determined the employee suffered an acl tear which required surgery. He began missing work on 8-22-2012

While retrieving High Line and other mining supplies in the Northwest Mains section of our mine that is in the process of being sealed, 6 falls and a large rib roll have been discovered. The falls did not interupt the ventilation in any of the bleeders. They range in size anywhere from 15' long, 20' wide to 70' long, 20' wide. All are at or above anchorage.

While traveling air course with KY inspector, a roof fall was discovered at 2520 spad. The fall is approximately 30' Long x 19' Wide x 6' High. Ventilation was not interupted. Fall was reported to MSHA field office and MSHA inspector was on site to inspect fall also. Fall located in bleeder area.
