Due to conflicting reports of injury location a medical review was conducted to determine the validity. On 8-24-11 the report indicated the injury was related to the accident. He started missing work on 8-4-11 and surgery was performed on 8-5-11.

On June 17, 2010 due to an occupational illness, as substantiated by medical records and reports, Jackie Thacker became unable to work because of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time.

EE was using a pry bar to hold a drive belt pully from turning while another EE was attempting to replace belts on the screen. The bar slipped causing EE to catch self. In doing so, EE had a whipping action with neck. Initially thought had whiplash but had cracked vertebra in neck. EE never mentioned the injury until 12/27/21 when EE went to Dr. EE had worked up to that point.

Employee had moved a Cat. 992 loader out of the way of the pit. He walked over to his dozer (147 ft) and began his pre shift of the dozer. He had walked around the front of the left corner of blade when he collapsed. He was found by the loader operator. CPR was initiated immediately and was continued to the hospital and until he was pronounced deceased.

He developed a sharp pain in his low back. Dr. at the E.R. examined him and said he had acute muscle strain, told follow up with family Dr., he saw family Dr. 8-5-09, and was taken off work. Fall was 55' X 20' X 5', there was no equipment involved, ventilation was not affected, passage of people was not affected. Area was breakered off, and we have no plans to rehab this area.
