Complaint What Happened On XXXX XXXX , a letter for validation of debt, amount presumably owed : XXXX , was sent to Medical Data Systems ask ing for a breakdown of fees including any collection costs and medical charges, a copy of my signature with the provider of service to release my medical information to them, and to cease any credit bureau reporting until debt was validated by me. As of this date XXXX / XXXX / XXXX , I was provided by Medical Data Systems an Itemized statement that included all procedures, not coded, which were performed including x-ray info which is a violation of the HIPAA act. I was not provid ed a statement from the original creditor that would allow release of my medical information to Medical Data Systems nor was reporting ceased to all XXXX credit bureaus. XXXX XXXX is the original creditor. I have never received a bill from this hospital and had insurance to cover all costs.
