The employer did not ensure that respirators were cleaned and disinfected using the procedures in Appendix B-2 of this section, or procedures recommended by the respirator manufacturer, provided that such procedures are of equivalent effectiveness: a) On or about April 5, 2019, in the crew job box the employees had respirators that had not been cleaned and disinfected and the employer did not ensure that cleaning had occurred between respirator uses in accordance with Appendix B-2.
