WE ARE EXCITED TO TRANSFORM THE CURRENT HEALTHY START PROGRAMMING DELIVERY MODEL IN LOUISVILLE, KY, INTO A CULTURALLY AND GENERATIONALLY RELEVANT, CLIENT-DRIVEN, EFFICIENT, FLEXIBLE, PROGRAM ROOTED IN EQUITY, LIVED EXPERIENCE, AND COMMUNITY ACCOUNTABILITY. THE HEALTHY START PROGRAM INCREASES ACCESS TO DIRECT AND ENABLING SERVICES FOR PREGNANT PEOPLE AND FAMILIES WITH INFANTS IN THE TARGET POPULATION AND COORDINATES THE PREVENTATIVE CARE AND EARLY INTERVENTION PARTNERS IN OUR COMMUNITY TO ELIMINATE DISPARITIES IN PERINATAL HEALTH, IMPROVE BIRTH OUTCOMES, REDUCE INFANT MORTALITY, AND PROMOTE FAMILY HEALTH AND WELL-BEING.
