Employee was hauling rock to the crusher under normal conditions. EE was traveling up a ramp from level two to level one and EE's truck stopped on the ramp. EE tried to back down the ramp and lost control of the vehicle resulting in the truck turning over on its side.

The miner was hand scaling a work face and removed a rock that hit the basket on man lift breaking it into several pieces. Some of the pieces hit their right arm and left hand. Miner said their back was also hurting because one piece of the rock hit their hard hat. Miner had a scrape on left arm and a small cut on right hand. Miner also had a red mark on their knee.

Mechanic driving a steel shaft out of bushing had a piece of metal fly off the shaft while striking it with a hammer. Mechanic had safety glasses and gloves on. Metal struck right arm and started to bleed. EE wrapped it up but it began to swell. EE went to Frankfort Regional Medical center after shift and they told EE to go to UK Sunday to get it removed.

Miner operating the JAW crusher called me at 11:36am saying they didn't feel good. I immediately responded and found miner in great pain. Miner was complaining of shoulder pain. I noticed miner was having symptoms of a heart attack and took them to the hospital. We arrived at the Frankfort hospital around 11:52 am.

Employee was cleaning out drill holes to be loaded. He was bent over and when he went to stand up felt something pop in his back. Was brought to the surface were he was taken to doctor. No x-rays or MRI were performed but was given muscle relaxer pills and pain pills. Was back to work on 6/7/12 but not able to do anything.

Employee was heating a bushing inside the arm on a hyd. hammer. It appears that the heat buildup caused the grease inside the arm to release back into the employees face. Employee had safety glasses on at the time of the accident. Employee had eyes rinsed out at the mine site eye wash station and was sent to the hospital.
